Dutch Springs Waiver

Every person entering Dutch Springs, whether scuba diving or not,
must fill out a waiver.  You may fill out a waiver when you arrive at
Dutch Springs, or you may print a PDF version of our waiver below,
and bring it to our admissions office
.

Please do not mail, fax, or e-mail waivers to Dutch Springs.
A waiver must be presented when purchasing an admission ticket.

Admission for Consecutive Days
If you purchase admission for consecutive days,
filling out and signing a waiver is only required on the first day.

Scuba Diving Waiver Information
Please read, print your name and address, date, sign and fill in:
Certifying Agency - PADI, NAUI, YMCA, etc.
Certification Date - from your C-card
Type of certification - open water, advanced open water, rescue, etc.
Card ID number - student number
If no student number is on your C-card, please use the last four digits of
your
Social Security number.  Temporary scuba certification cards are accepted.

Certified Scuba Divers:
Once a waiver has been filled out and signed, a sticker will be placed
on your C-card indicating that a signed waiver has been submitted.
As long as you show your C-card, with our sticker, another waiver
does not have to be submitted during our calendar year.

Non Scuba Diving Waiver Information
You must fill out and sign a waiver before entering Dutch Springs.
Please read, print your name and address, date and sign the waiver.

Children under 12 years old must be accompanied by an adult.


Management Reserves the right to ask for picture identification
and/or a scuba diving certification card.
